    Kindergarten Health Examination Report Form

    Have you had your child's doctor complete the mandatory Health Examination Report? Here is all you need to do:

    1. Click the link to view the form: Health Exam Form
    2. Print the form
    3. Take the form to your kindergartener's doctor appointment. 
    4. Give the form to the doctor to complete.
    5. Return the completed form to the school office.
