This request shall clearly state the details of the amount of sick leave requested.  Appropriate written verification of the catastrophic illness or injury must be included with the request.  A Sick Leave Bank Committee shall consider the request of the unit member.  The maximum number of Sick Leave Bank days allowed to be withdrawn by one unit member for a single catastrophic injury/illness shall not exceed fifty (50) days.
